What is Interstitial Cystitis?

Interstitial Cystitis (IC), also known as painful bladder syndrome, is a long-term condition characterized by bladder pressure, bladder pain, and pelvic pain. The pain ranges from mild discomfort to severe. The bladder, which stores urine, is affected by chronic inflammation, leading to its reduced capacity and painful urination. IC symptoms can come and go or persist over long periods. Unlike regular urinary tract infections, IC is not caused by bacteria, and antibiotics are often ineffective. It primarily affects women but can also occur in men.

Causes of Interstitial Cystitis

The exact cause of Interstitial Cystitis is not known, but several factors are thought to contribute. These include defects in the bladder lining, which allow irritants to penetrate and inflame the bladder wall. Autoimmune disorders, allergies, infections, or previous pelvic surgeries may also increase the risk. Some researchers believe nerve dysfunction in the pelvic region can heighten bladder sensations, causing pain. Hormonal changes and genetic predisposition are also considered possible causes. The multifactorial nature of IC makes diagnosis and treatment complex, requiring a personalized approach.

Diagnosis of Interstitial Cystitis

Diagnosing Interstitial Cystitis involves ruling out other conditions that cause similar symptoms. A healthcare provider may begin with a detailed medical history and physical exam, focusing on the pelvic region. Urinalysis and urine culture help eliminate the possibility of infection. Cystoscopy, a procedure using a thin tube to look inside the bladder, may reveal bladder wall inflammation or ulcers. Urodynamic tests assess how well the bladder holds and releases urine. Symptom diaries and questionnaires are often used to track frequency and intensity of symptoms.

Symptoms of Interstitial Cystitis

Symptoms of IC vary but typically include bladder pressure, pelvic pain, and an urgent, frequent need to urinate  even in small amounts. Pain may increase as the bladder fills and may be temporarily relieved after urination. Some patients also experience pain during sexual intercourse, and in women, symptoms may worsen during menstruation. Unlike a typical bladder infection, IC symptoms are persistent and not resolved with antibiotics. Flare-ups can be triggered by stress, certain foods, or physical activity.

Treatment for Interstitial Cystitis

Treatment for Interstitial Cystitis is individualized and focuses on symptom relief. Options include lifestyle changes like dietary adjustments to avoid trigger foods, physical therapy for pelvic muscles, and bladder training. Oral medications such as antihistamines, antidepressants, and pentosan polysulfate sodium can reduce bladder inflammation and pain. Bladder instillations deliver medications directly to the bladder lining. In severe cases, nerve stimulation or surgery may be recommended. Consistent follow-up and symptom monitoring are essential to find the best treatment plan for each patient.
