What is Hemophilia ?

Hemophilia is a lifelong bleeding disorder where the blood doesn’t clot properly due to the absence or low levels of clotting factors. The two main types are Hemophilia A (factor VIII deficiency) and Hemophilia B (factor IX deficiency). Causes of Hemophilia

In about one-third of cases, the condition results from spontaneous mutations without family history. Rarely, hemophilia can be acquired later in life due to autoimmune responses that attack clotting factors, often triggered by cancer, pregnancy, or certain medications.

Diagnosing hemophilia begins with a thorough evaluation of the patient’s personal and family history of unusual or prolonged bleeding. Blood tests are then performed to assess clotting times and measure specific clotting factors. The most common diagnostic tools include aPTT (activated partial thromboplastin time), PT (prothrombin time), and factor assays. In cases of suspected genetic inheritance, DNA testing may be done. Early diagnosis is key, especially in families with known hemophilia history, to prevent joint damage and life-threatening bleeding.

The symptoms of hemophilia vary based on severity. Common signs include prolonged bleeding from cuts or injuries, frequent nosebleeds, and excessive bruising. Bleeding into joints, which causes pain, swelling, and stiffness, is another hallmark symptom. Severe cases may experience spontaneous bleeding without injury. Blood in urine or stool and bleeding after vaccinations or surgeries are also common. In infants, excessive bleeding from gums or prolonged bleeding after circumcision may be the first sign. Early detection is vital for effective treatment.

Treatment of Hemophilia

Hemophilia is treated primarily through replacement therapy, where the missing clotting factor is infused into the bloodstream. This can be administered as needed (on-demand) or regularly (prophylactic). In mild Hemophilia A, desmopressin (DDAVP) may stimulate the body to release stored factor VIII. Antifibrinolytics help prevent clots from breaking down. Gene therapy and newer medications like emicizumab are also improving outcomes. Treatment is tailored based on type, severity, and patient age, with ongoing care to manage joint health and prevent complications.
